What is a Cart and Online Inquiry Request Form?

Residents of Arizona can use the online Cart for browsing and selecting items from the Assistive Technology Demonstration and Program Inventory, and complete the loan inquiry request form online to send an email to AzTAP staff to request more information about and/or borrow the items in their cart.

As you find items you may want to borrow, click the link, that reads, "I'd like to find out more information and/or borrow this device - add it to my "cart". Your items (up to 5) will be placed in the cart. 

When you are finished browsing the Demonstration and Program Inventory, click on the link, "Complete the Inquiry Request Form Online." Answer a few questions, provide your contact information and the items from your cart will be included on your inquiry request that is emailed to AzTAP staff who will then contact you regarding your inquiry.
